PAIN ASSESSMENT IN PATIENS, IN WHOM VASCULAR PROCEDURES USING PROSTHETIC GRAFTS ARE INDICATED, BASED ON THE DYNAMIC MODEL OF PSYCHOLOGICAL PROCESSES IN CHRONIC PAIN

Abstract

The aim of our research is to create an adequate preoperative education in order to reduce anxiety of patients, therefor it is necessary to understand their emotional experiences, cognitive processes and behavioral aspects as well. This article presents the development of a measure of pain in patients in whom vascular procedures using prosthetic grafts are indicated. The test battery is based on the Dynamic Model of Psychological Processes in Chronic Pain primarily used in clinical practice. Dynamic Model includes 8 consecutive stages and there can be some degree of overlap. Three were included in our research. 1) Pain beliefs and attitude 2) Trigger processes – pain and pain evaluation 5) The second affective stage (includes coping strategies). Pain beliefs and attitudes are assessed before operation by PBPI (Pain Beliefs and Perception Inventory). Intensity of pain is evaluated by VAS (Visual Analogue Scale) and NRS (Numerical Rating Scale). Pain intensity, Unpleasantness and Quality of life are measured by VAS every day during hospitalization. Pain intensity is also measured by NRS several times a day and it is supplemented by figure drawing which enables exact pain localisation. Affective characteristics are assessed by STAI before and after operation (The State-Trait Anxiety Inventory). Coping strategies are diagnosed after operation by ABI-P (Anxiety Coping Inventory - related to physical threat). Basic clinical characteristics of our study sample (N = 33) are summarised in this paper.
