MULTIDISCIPLINARY TEAM IN HOME HOSPICE CARE

Abstract

Hospice care is a specialized, integrated form of health, social and psychological care provided to patients of all indication, diagnostic and age groups for which a reasonable life expectancy is judged to be less than six months based on reasonable medical judgment and for which active treatment has been discontinued. It is essential to treat pain and care for other symptoms of the terminal stage of the disease, as well as to address the psychological, social and spiritual needs and problems of the sick and consequently their family members. In home hospice care, teamwork is necessary, which includes, in addition to health care professionals, a clergyman, a psychotherapist, volunteers, but also the sick and family-members and his friends. Methods: The method of quantitative research was used, the content analysis of the documentation of the patients of the home hospice. We have analyzed 35 medical progress notes. Individual interventions were coded to cover and saturate selected areas provided by the multidisciplinary team. Results: Hospice patients were most visited by nurses during the day and in almost 59.1 %; doctors in 15.5 %, mostly once a week, if there were no complications and deterioration of health. Visits of social workers, caregivers, psychologists and clergymen were less frequent, on average once in the period of care provided, according to the needs and requirements of the patient and his loved ones. Another form of contact was phone calls, the most common among patients and nurses, in almost 25 %. Conclusion: The field team consists of professionals who provide individual and comprehensive care to terminally ill patients and their families. Supports patient relationships with family and other close people. Offers expert advice.
