ACUTE KIDNEY INJURY IN SEPSIS IN THE ELDERLY

Abstract

Background: Sepsis is a severe medical condition associated with multiple organ failure. Acute kidney injury is a severe and frequent complication in patients with sepsis, especially in the elderly, causing prolonged hospitalization and additional costs. Aim of study was to evaluate clinical features, risk factors, evolution and costs of AKI induced by sepsis in elderly .Material and methods: A retrospective study was performed over the past 6 months based on medical records in our Nephrology department and Nephrology ICU on patients who fulfill Kidney Disease Improving Global Outcomes 2012 criteria for AKI associated with sepsis. Standard demographic and clinical data were collected. Inflammatory tests (leucocyte count, ESR, CRP), kidney function (urine output, serum creatinine, urea, e GFR) and vital signs were noted. Sepsis severity was assessed using the Acute Physiology and Chronic Health Evaluation (APACHE II), Sequential Organ Failure Assessment (SOFA) and Simplified Acute Physiology Score (SAPS 2). The duration of hospitalization and related costs were noted. Elderly patients were defined as those aged over 65 years. We split the cases in two groups: 65 years and we compare the results. Conclusions Acute kidney injury is frequent associated with sepsis in elderly patients, especially those with additional risk factors such as cardiovascular disease, urolithiasis and chronic urinary catheterization. The presence of AKI in elderly lead to longer hospitalization and additional care costs compared to younger patients.
